Vendor Information & Approval
The DMHS Girls Basketball Booster Club works with businesses and service providers throughout the year to support our girls basketball program.
To maintain strong financial controls, protect our student-athletes, and ensure we are working with qualified and appropriate businesses, vendors may be required to complete our approval process before providing paid goods or services.
Vendor Requirements
Depending on the services being provided, vendors may be asked to provide:
- A completed IRS Form W-9
- Business name and contact information
- An invoice or written estimate
- Proof of insurance, licensing, or professional credentials when applicable
- References or additional information when appropriate
Vendors Working With Student-Athletes
The safety of our student-athletes is a priority.
Vendors who may have direct interaction with players, including photographers, trainers, instructors, transportation providers, or other service providers, may be subject to additional review.
This may include verification of identity, business history, references, insurance, professional credentials, applicable school or district requirements, and background screening when appropriate.
Vendor approval does not authorize unsupervised access to any student-athlete.
Approved Vendor List
Once approved, applicable vendors will be added to the DMHS Girls Basketball Booster Club Approved Vendor List.
Approval does not guarantee future work or purchases. It confirms that the vendor has completed the Booster Club’s required review process and may be considered for authorized work.
